CARE FOR OLDER PEOPLE We provide high quality care/support services to clients with diverse needs who are living in their own homes. Before we create a care support package one of our staff will visit to discuss the person''s needs and agree a care/support plan with you. The plan will be individually tailored to meet the clients needs and wishes. If an individual is already receiving care under an agreed plan, which was arranged between themselves and another local agency (e.g the local authority social services or local health services) we will adhere to that plan accordingly. This situation may occur if the client has chosen to organise their care through the direct payment system. We pride ourselves in being flexible and adaptable to changing needs. All plans are regularly reviewed with clients and if appropriate their family and friends. As people grow older they can become vulnerable within their home as a result of health problems and mobility issues. Whilst these problems can occur over a period of time people can also experience a sudden change of crisis in their home or perhaps that of a relative. We know it is important to work in partnership with everyone in the client''s circle of support. This includes with agreement family members who have a wealth of knowledge and understanding of the individual, which can be shared with us to ensure we provide appropriate and consistent care and support. Our services are available logistics allowing from fifteen minutes a week to 24 hours, seven day a week. All our services can be provided directly to the client via the direct payment scheme. ADULT WITH SUPPORT NEEDS We assist adults with learning disabilites. Physical disabilities and or mental health problems to live as independently as they can within their communities. Southern Family Care work closely with other professionals and extended families to ensure our services compliment the overall support provided to individuals within their home. Our approach in partnership is to assist individuals to gain or re learn skills which will assist them to achieve higher levels of independence and to reach their own personal goals in life, whilst receiving the support they need to live safely within the community. Using a person centred approach that places the needs and preferences of the service user at the very heart of the services we offer. We ensure that care plans not only addresss issues of welfare and safety but they are written monitored and reviewed in partnership with the individual service user and that the focus always remains on addressing an individual''s needs, and aim to achieve the outcomes they seek in becoming happy, confident and independent citizens. INDIVIDUALS WHO MAY BENEFIT FROM OUR SERVICES We are able to support young adults in the transistion from childrens services or living with parents, to setting up their own home for the first time. We also support adults with a learning disability and or physical disability make the transition from residential or hospital care to live within the community either on their own or with the close support of others. Southern Family Care support adults with a disability living with their families, by offering families and carers support including community based activities, facilitating short breaks and providing care and support within the home. We are also able to assist individuals with mental health issues to be more fully supported in the community, by providing both practical and emotional support in partnership with other professionals and agencies. CHIDREN AND FAMILIES Southern Family Care work to promote child safety and good parenting working in unison with families,education and social services. We work with key support agencies and we are able to engage directly with parents and vulnerable children to address and alleviate many of the problems which can arise from poverty, ill health and disruption within families. Children and families benefit from the practical support we can offer and in providing such support children benefit from a sense of security that may of previously been missing. Children also benefit from the planned activities we organise which help to build their confidence enabling them to achieve their true potential. Southern Family Care is also able to use its expertise to support children with disabilities and their families in dealing with child care and accessing support services. We support young people in the transistion to adulthood when issues of mental health, self-harm, drugs, relationship difficulties, offending and unemployment can all potentially have a devastating impact unpon a young person at a critical time in their lives. INDIVIDUALS WHO BENEFIT FROM OUR SUPPORT Parents experiencing difficulties in their own lives that have a negative impact upon their children. Children who need and would benefit from additional support to access education and community based activities. Parents who need practical advice and support in parenting or who require additional support to facilitate contact with a child/children living with another parent or alternative carers. Young people who need additional support in dealing with what can sometimes seem overwhelming challenges of becoming independent including care leavers. Services We Offer For Children and Families Parenting skills and practical support for parents including short breaks. Activity based support services to assist families and agencies facilitate contact and planning for children. Practical home support and community based support for families with a child with a disability. We can also provide these services through the direct payments scheme. HIGH LEVEL FAMILY SUPPORT The aim of our high leve family support service is to support and encourage families to make a positive and lasting change. Our aim is to prevent the issues that could lead to such measures as foster care of youth justice procedures. Children within these families may face a wide range of issues that could include deteriorating physical and mental health, domestic related violence, drug and or alcohol abuse, a lack of social skills. Southern Family Care focus on keeping the family together. We do this by preventing anti-social behaviour, increasing the self esteem of vulnerable parents, improving school attendance and preventing homelessness. Every child and family is different with the correct support the family as a whole can make a positive contrubution to their local community. At Southern Family Care it is our stongly held belief that support should be organised and delivered in a way that respects the knowledge and skills you have gained from your own experience. We will listen and work in partnership with you to design an individually tailored support package that reflects the needs of your family and your child, this could include some fo the following services; Helping you to be aware of all your social, family and community assets. Organising a support package that you understand clearly Delivering the right support at the right time for example bed times and helping to get your children ready for school. Providing home support this could include helping you with daily tasks such as changing beds and tidying up.
